Report Writing

  1. Write the report as soon after the incident as possible.
  2. The report must be Factual.
  3. Always assume the reader knows nothing of the incident
  4. Never use jargon or slang.
  5. Write in black ink as they will need to be photocopied.
  6. If you make a mistake draw a line through the words.
  7. Keep a precise record for yourself. (Medical or Legal)
  8. Record any witnesses on the incident form.
  9. Fill in all the boxes. (Time/location etc)
  10. Always draw a map.
  11. If you need more space – get some more paper.
